$10 for S/H, cash/check prefered, 3% credit card fee will apply to this program.
Excludes controlled substances, this program is for generic prescription drugs only with greater than 90 day supplies and cannot exceed 365 days (1 year).
This program cannot be used with any other program. However, you may be able to seek reimbursement from your insurance company through self-submission.
